Our mission is to establish an all-embracing ecosystemic network comprised of committed community stewards who provide individually designed practical and emotional support to individuals experiencing minimal community substructure.
Why Aftercare Support?
[Whole Person Centered Approach]
Aftercare Support is a whole person-centered support service offered by Cascadia Deaf Nation (CDN) with the primary goal of enhancing the quality of life for and with BIPOC Deaf community members (including D/deaf, DeafBlind, DeafDisabled, Hard of hearing, and Late deafened individuals). We aim to address the gaps that have left these individuals feeling underserved and guide them to meet their needs. Through a proactive approach rooted in mindfulness, stewardship, and transformative justice, our Aftercare Support service seeks to support and uplift these community members on their lifelong journey toward an improved quality of life.
The Community Care Timeline
This is the moment where an individual has not received supportive service during life transitional period.
This is the time where an individual starts to receive or is receiving not received supportive service during life transitional period. Ex: DVR support, DV advocacy, re-entry, etc.
This is the mentoring or community support stage where an individual receives the support in navigating opportunity to strengthen own self-advocacy or become confident in making healthy decisions to improve own quality of life. This is where we come into the picture with our resource network.
The person is not alone in preparing for own death or for a family's member's death
Sustain support during medical emergency and support with maintaining own health at its optimal best.
Community centered mutual aid covering unexpected expenses during situational crises
Support in providing sustainable housing resources including housing cooperative opportunities
Whole person centered mentoring to support with building health self-advocacy & self-activism while encouraging inner work in both personal and professional areas.
Support with facilitating critical information and guiding the person to know their human rights on a deeper level.

At our Aftercare Support service, we are committed to creating a thriving community founded on healed human connections, overall well-being, access to nutritious foods, secured housing, and a sense of purpose. While financial resources are essential for sustaining a healthy community, we understand that there are other critical factors contributing to its ability to thrive equitably. Our main focus is on promoting holistic well-being and fostering a strong sense of belonging within our community.
Shared Knowledge..
Through our understanding of collective power, knowledge sharing intends to advance the spread and strengthening of fair representational information and resources. This enables individuals to improve their practical and creative thinking abilities while also reaping the benefits of Aftercare Support services. The stewardship role in this endeavor is aimed at fostering personal growth through the exchange of knowledge.
